Direct Admissions

The Direct Admission Program is designed to provide varying treatment levels to avoid the hospital for healing and recovery. The Direct Admission Program is administered at our modern rehabilitation facilities and results in:

  • Lower cost to the patient
  • Better Outcomes
  • Higher Patient Satisfaction

A referring physician will work with the patient to determine the best plan of care, matching the severity of the illness to the services required for recovery.

Step 2 – If the patient meets the program qualifications, our team will begin working immediately to obtain the appropriate files, arrange for transportation and facilitate a seamless transfer into our facility.

Step 3 – Patients can be admitted from home, physician’s office, clinic, emergency department or urgent care center.

    Indications for Care

    The Direct Admission Program is designed to treat various levels of care to help patients avoid the hospital for healing and recovery.

    Services Include:

    • Non-Emergency Illness or Injury
    • Simple Fractures
    • Lacerations, Rash, Skin Ulcers, Cellulitis
    • Injuries or Illness Resulting in Loss of Mobility or Independence with Activities of Daily Living
    • Deconditioning/Decline in Function
    • Infections
    • Dehydration

    Rehabilitation Care

    Patients will be cared for by highly skilled physicians, registered nurses and rehabilitation personnel specializing in clinical medicine, physical and occupational therapies, infectious disease, and wound care.

    Services Include:

    • Cardiac Telemetry Monitoring
    • In-House Dialysis
    • Intravenous Medications and Fluid Replacement
    • Oxygen and Respiratory Services
    • Telemedicine
    • Wound Care
    • X-Ray and Ultrasound

    Home Care

    Patients will be set up for Home Care services, if indicated, upon discharge from the rehabilitation program.
